questionnaire?
A. Paradigm Group/Healthy AIM licenses the health risk questionnaire software
from Wellsource, Inc. Wellsource is a Portland, Oregon health risk assessment
company that has been in business for over twenty-five years.

Who has access to my personal health information (PHI)?
A. Neither your employer nor health plan have access to your PHI. Your PHI is,
however, aggregated in depersonalized form and provided by Paradigm Group to your
sponsor in a profile of the aggregate risks of the covered population. For example,
the Aggregate Report may state that a certain percentage of the covered population
has below-target fitness levels.
PERSONAL HEALTH INFORMATION ACCESS:Your PHI is available in whole or in part only to the following entities: Paradigm Group/Healthy AIM and Wellsource, lab/phlebotomy service subcontractor for Healthy AIM, Quest Diagnostics, which is the national lab subcontractor that tests your blood sample.
For the minority of participants whose health status warrants intense professional
assistance, Paradigm may provide PHI to the Case Management and EAP service
subcontractors as applicable for further evaluation and possible intervention.
Participants independently and privately decide whether or not to participate
in any services offered by these service providers. |
Q. Why do you need my email address?
A. Your email address may be used for identification verification, to receive confirmation, updates, and reminders regarding wellness services, for population tracking, and to receive additional wellness information—as a service offered by your employer or individually requested. As with your PHI, your email address is available only to the authorized service providers mentioned above. You will not be solicited through email.
Q. Why might you need
my Social Security Number?
A. Your SSN may be your health plan ID upon which the wellness program depends
for registration and PHI maintenance (particularly with respect to assuring that
no party other than you and the PHI-authorized service providers mentioned above
may access your health status profile).
Q. Is this HIPAA compliant?
A. The Healthy AIM program is fully compliant with HIPAA’s Privacy and Security provisions.
Q. Who/what is Healthy AIM? Paradigm Group? Health InfoNet?
A. Paradigm Group is the umbrella name for two health service companies: Health
InfoNet and Paradigm Consulting. Health InfoNet is the corporate entity through which Healthy
AIM services are incorporated into wellness programming offered by your health plan or employer.
Paradigm Group is based in Billings and has been in business since 1993.
