  • Choose your location from the Drop-Down Menu, and follow the directions below

    • Your username is set to be a capital P for Participant, or an S for plan-covered Spouse, followed by employee ID number with no dashes or spaces
      • This is not your SS
  • Password
    • Your password is confidential and was selected upon initial login

Once logged in, follow the instructions below to access your Health Assessment:

  • Go to My Health Tools in the lefthand column
  • Choose the Online Wellness Library for useful health information and resources
  • Choose the Personal Wellness Profile link to review your Personal Wellness Profile,
    then
    click on the Completed Assessment box
